If you wish to take books out of our library please print out this form, complete it and send it to the Branch librarian at parnold@rschicago.org or Paulette Arnold, Librarian, Rudolf Steiner Branch, 4249 N Lincoln, Chicago, IL 60618.

If you sign up to use the Library, you will be added to the mailing list by the Librarian.
